- Fill Out The Appropriate Enrollment Form Below.
- Send a DVD up to 12 minutes in length. ( If longer it will be edited) Youtube video now accepted as well.
- Please mark your DVD clearly with your name and phone number.
- Email two photos in JPEG format or you can send them with your DVD.
- Note: DVD's will NOT be returned.
- Mail your enrollment form along with a check for $49.99 to
